The 90 days
A marketing lead at a two-location US med spa, doing about $350,000 a month, joined up five systems for 90 days, December 2025 to March 2026: the CRM, the phone system, Google Analytics, Aesthetic Record and Meta. He posted the results on r/MedSpa in March 2026. He also runs an aesthetics agency, so weigh it knowing that.
Organic search brought 47% of the site's 9,766 visits. The CRM credited Google with 20 leads. Over the same period the spa took 1,444 first-time inbound calls.
Why the CRM misses it
His explanation: patients researching treatments don't fill in forms. They call. A search visit that ends in a phone call never shows up as a Google lead, so search looks like it does almost nothing.
The same report found Meta leads rarely booked right away and usually needed three to five calls back. His summary: Meta creates demand, Google validates the clinic, and the phone converts the patient.
What goes wrong when you can't see it
An owner who can't see search working cancels it, or keeps paying an agency whose report shows clicks instead of patients. Zenoti makes the same point about agencies: one that can't say how many patients your spend produced is measuring activity, not return. How to count it
Give the website its own phone number, so calls from the site are counted apart from calls from your Google listing and your ads. Record where every form inquiry first came from. Then once a month, match inquiries to bookings and payments in your booking system by phone number or email.
Count invoices, not line items. Some booking systems export one line per product or unit, and adding those up makes revenue look larger than it was.
And AI search
Patients now ask ChatGPT and Google's AI answers which clinic to try. Those answers are built from what's written on the web, your site and your Google listing included. A treatment page that states the price unit, the injector and what the first visit is like gives them something to repeat. Nobody can promise you'll be named, and nobody honest does.
What this doesn't tell you
It's one clinic's report, posted by someone who sells marketing. The pattern, searchers calling instead of filling in forms, is worth checking in your own numbers before you change anything.
